Request License Verification

Anyone can quickly and easily verify a license by doing a name search directly on the Board's website under "Verify a License."

All Licensees - MD/DO, PA, GC, and ND

All licensees under the NDBOM needing verification sent directly to a U.S. licensing board must utilize VeriDoc. Go to VeriDoc to request the verification of your North Dakota license to another licensing board. All verifications processed through VeriDoc are considered primary source verifications under National Committee for Quality Assurance standards.  You will pay by credit card and the verification will be electronically sent to the state board requesting verification. Verifications obtained through VeriDoc are accepted by all state licensing boards in lieu of any form they might have. 

If you need a license verification sent to a location outside of the U.S., you will need to mail your request to the NDBOM along with a check or money order for $30.00. Please clearly indicate the address where the verification is to be sent. NOTE: this is only for locations outside the U.S.  If you send a check/money order and request for verification for a jursidiction within the U.S., it will be shredded and you will be referred to VeriDoc  - which will result in delays.
